Care Coordination Resource of Alaska is a program of Alzheimer's Resource of Alaska (ARA) and we are looking for two dedicated Care Coordinators to join us in enriching the lives of Alaskans in need. This is a great opportunity to work in our Juneau location, full or part time.

Responsibilities include:

• Assess client/family needs
• Identify and coordinate the delivery of services
• Screening, assessment, service coordination, monitoring, follow-up and advocacy 
• Liaison between community members, service providers, clients and state representatives

Requirements/Qualifications

• Bachelor's degree in social work, psychology, nursing, health, human services or closely related field. (Some of the education requirement may be substituted for additional work experience)
• Minimum of one full year paid work experience in human services.Training is possible for the right candidate.
• Certified by the State of Alaska to provide Medicaid Waiver care coordination
• Ability to pass a state and federal criminal background check
