Contact John French (John.French@uas.alaska.edu) to have the form creation page added to your set of asset factories in the CMS.

To use the form, you simply edit the form page to  create the input boxes and groups. You may even add a title and explanation of the form at the top of its page. The the cms makes it look good and function in a compliant fashion.
