Ray Watkins handles key and electronic access requests; he can be reached at 796-6495.

Electronic Access/Key Request Form - to request access to UAS facilities submit the electronic access/key request form complete with dean/director signature to our office via inter-campus mail or fax (796-6499). 

Identification Card Request & Change Form - to change or terminate access already established for a building submit the change request form complete with dean/director signature to our office via inter-campus mail or fax (796-6144).

Please be aware that there is a $25 fee for a lost key.  When terminating employment with the university, keys must be returned to Facilities Services.
