You may be asked to account for the information you put on your FAFSA This process is called Verification and requires you to submit a number of documents to the Financial Aid Office for review
Outstanding Requirements
You will receive a letter from our office, informing you of anyrequired documents needed for verification. You can review your outstanding requirements at UAOnline.
You may be asked for a signed copy of completed tax return(s) and W-2's and a signed copy of the University of Alaska Southeast Verification worksheet.
If you don't have a copy of the required tax returns(s) you must request a free tax transcript from the IRS at 1-800-829-1040 (listen for the option which says "... for questions about your personal tax records ..."). Or, send the IRS Form 4506-T. The transcript should arrive within 10 to 15 days.
You may also be asked to complete additional forms. Some examples are listed below:
- Verification Worksheet
- Low Income Form
- Copy of your birth certificate,
- Copy of your Social Security card,
- Copy of your marriage certificate,
- Military Leave Earning statements (LES),
- Copies of Aid to Families with Dependent Children printouts,
- Selective Service verification,
- Proof of citizenship.
