Rebuild/Replacement Form
- Please read, complete and submit the Rebuild/Replacement form.
- The Rebuild/Replacement form will be reviewed within one business day by the Helpdesk.
- The Helpdesk will notify you via email when the form is approved. If the Helpdesk has any questions regarding the form they will contact you.
- After your Rebuild/Replacement form is approved, you will need to reserve the checkout laptop from the Computer Lab (796-6521) for the day you would like the work done, if available.
- A work order is created when you you pick up the Rebuild/Replacement laptop, or have it delivered on reservation day. Your computer must be available to be worked on at this time. Please do not pick up the checkout laptop before you are ready to have the work done.
- After you have checked out the Rebuild/Replacement laptop the rebuild process will be completed by the end of the next business day - no later than 4:30pm.
- Your computer is returned to you by an NDS tech and the checkout laptop is returned to ITS.
