Purchase Request Form
This Purchase Request Form is intended for use by University of Alaska Southeast students, faculty, and staff only. Use this form to suggest an item that the Egan Library should purchase. Please check the Library Catalog before submitting your request, to see if the Library already owns the item. The Library generally does not purchase duplicates of items in the collection. The Librarian Liaison for your subject area may contact you about your request. We will always let you know if we decided to purchase or were unable to purchase the item you requested. If the decision is to purchase, we will also notify you when the item is available for check-out. Please provide the following information:
