Health assessments worth $100
The deadline for turning in the University of Alaska Personal Health Assessment is November 15, 2007.
Each employee and covered spouse completing the assessment will receive $100 checks in the mail by December 15, 2007. You can complete the paper version mailed to your home or the online version available at:
