The plan officially kicks off November 1 and employees will be receiving information in their home mailboxes on how to qualify for next year’s rebates. Activities to qualify must be completed by April 30, 2015. There are lots of ways to qualify including getting an annual eye exam, dental exam, immunization (including flu shots), completing Healthyroads Lifestyle coaching sessions, plus logging a wide variety of physical activity.
