Writing Center Feedback Form
This form is filled out by you and your writing tutor and is submitted to your instructor as proof of your visit to the Writing Center. Please note that this is a general form and many writing instructors have their own forms.
Placement Test Administration Agreement Form
This form is filled out by you and your proctor if you plan to take placement tests before arriving inJuneau.
Test Information Form
This form is for instructors who want to use the Testing Center. Please fill out this form and send it to us along with your exams.
Examination Certification Form
This form is for instructors who use the Testing Center. We are required to have every student sign this form prior to taking the test. It would help us greatly if you made this form the cover of your exam; if you don't do so, a copy will be attached by a Testing Center proctor.
